Here’s how Golden State Recuperative Care handles client referrals.
STEP 1
REFERRAL
A partnering healthcare facility or hospital sends a referral to make arrangements for recuperative care.
STEP 2
INTAKE
Our care manager evaluates the clients needs and formulates the appropriate care plan.
STEP 3
ACCOMMODATION
The client enters our facility and receives a welcome kit and a semi-private room.
STEP 4
CARE
The client then receives medical oversight, medication management, meals, transportation, and other supportive services.
STEP 5
LINKAGE
The care staff links the client to primary care, CES, mental health counseling, and other related social services to achieve independence and secure temporary/permanent housing.
